What has happened is that the industry has consolidated and learned how to extract rents everywhere and anywhere they can. Primarily by contractually making sure there is zero transparency.
If you can’t see the numbers that everyone else pays, and what they are paying for , they can lie to you and say you are getting a great deal.
When tech companies come in to “optimize”, they optimize the rent taking. Not helping patients
Plus there is regulatory capture
The fix for healthcare isn’t complicated.
Work outside the companies that add the complications
What you will find is those biggest companies, in particular PBMs and their captive insurance companies, and the complexities they add, are not needed.
There are smaller , simpler companies (like @costplusdrugs ) that can do the same job or more
Big self insured companies can set up direct contracts with providers. Instead of using the biggest insurers, they can use independent claim adjudication. (Is it really insurance if they do everything possible not to pay claims ?)
They can contract directly with pharmaceutical companies for super high priced specialty therapeutics
Providers know that all the contract complexities, pricing and network games played with big insurers is ruining the quality and cost of care, but they are terrified they have no choice.
They should be receiving advances from self insured companies as a means of mitigating the providers’ revenue risks rather than that money going to insurers.
In exchange, for providing providers cash advances , they should receive flat Medicare reference pricing , which can also lead to less paper work and administrivia for doctors, fewer claim denials and less need for pre approvals
But none of this happens because no one trusts anyone in the healthcare supply chain.
If self insured companies moved their business away from the biggest players to smaller, transparent players, the industry would simplify quickly. To the betterment of everyone.
