🔹 AAV = 3 intertwined diseases:
GPA 🩸 | MPA 🧬 | EGPA 🫁 — overlapping but distinct by organ pattern + ANCA specificity (PR3 vs MPO).

🔹 Diagnosis dilemma:
No gold standard. 10–50% can be ANCA-negative! Clinical overlap & mimickers (e.g. infections, autoimmune overlap) delay treatment.

🔹 Improved classification (2022 ACR–EULAR):
Combines phenotype + ANCA type → better trial design & prognosis.
✅ GPA: PR3-ANCA, ENT/lung
✅ MPA: MPO-ANCA, renal dominant
✅ EGPA: Asthma + eosinophilia ± MPO-ANCA

🔹 Tools that matter:
📊 BVAS → disease activity
⚙️ VDI / CDA → organ damage
🧍 AAV-PRO → patient’s quality of life
👉 Future = integrated composite scoring (activity + damage + PROs).

🔹 Prognostic pearls:
• Remission in 70–90%, but relapse in ~50% within 5 yrs.
• PR3-ANCA → ↑relapse risk 🔁
• MPO-ANCA → ↑renal & mortality risk 💀
• AKRiS = best kidney outcome predictor.

🔹 Therapeutic evolution:
💊 Rituximab, Avacopan (C5aR blocker), anti-IL-5 transformed survival.
💡 Reduced-dose steroids = fewer infections + less toxicity (PEXIVAS).

🔹 Emerging trend:
Precision medicine 🧩 integrating genetics + biomarkers + ANCA subtype = personalized treatment & outcome prediction.

🔹 Bottom line:
AAV care ≠ just immunosuppression — it’s dynamic disease stratification + patient-centred metrics + relapse prevention.
