▫️The authors later call this location the distal femoral triangle.
▫️So why continue calling it an ACB?

▫️It does not confirm catheter location inside the AC.
🟠 B. 𝗧𝗛𝗘𝗡 𝗪𝗛𝗬 𝗧𝗛𝗘 𝗗𝗘𝗟𝗣𝗛𝗜 𝗖𝗢𝗡𝗦𝗘𝗡𝗦𝗨𝗦?
▫️FT and AC are anatomically different locations.
▫️Terminology was standardized for a reason.
▫️Yet distal FTB evidence still enters literature as “ACB”.
▫️Today’s mislabeling becomes tomorrow’s meta-analysis.
FTB → labelled ACB → pooled as ACB → misleading evidence.
🟡 C. 𝗕𝗨𝗧 𝗪𝗛𝗔𝗧 𝗜𝗦 𝗧𝗛𝗘 𝗣𝗔𝗜𝗡 𝗧𝗔𝗥𝗚𝗘𝗧?
▫️The paper itself states that the ACL receives articular branches from the tibial nerve.
▫️FNB mainly covers anterior femoral-derived innervation.
▫️A distal FTB again mainly targets anterior/anteromedial pathways.
▫️Neither directly addresses the principal ACL neural pathway.
▫️A true distal ACB has a more logical pathway toward distal/posterior knee innervation.
▫️Injection level therefore matters.
🔵 D. 𝗦𝗧𝗔𝗧𝗜𝗦𝗧𝗜𝗖𝗦 𝗖𝗔𝗡𝗡𝗢𝗧 𝗙𝗜𝗫 𝗪𝗥𝗢𝗡𝗚 𝗔𝗡𝗔𝗧𝗢𝗠𝗜𝗖 𝗖𝗟𝗔𝗦𝗦𝗜𝗙𝗜𝗖𝗔𝗧𝗜𝗢𝗡
▫️CFNB was not superior for the primary outcome.
▫️That does not prove equivalence.
▫️The >50-MME result was exploratory.
▫️Quadriceps strength was not measured.
▫️Yet motor preservation is central to the FNB-ACB debate.
🔑 𝗞𝗘𝗬 𝗧𝗔𝗞𝗘𝗔𝗪𝗔𝗬𝗦
▫️Mid-thigh should not automatically mean ACB.
▫️Block name must follow anatomy—not tradition.
▫️ACL pain targets matter more than block labels.
▫️Misclassified blocks create misclassified evidence.
▫️And meta-analysis only amplifies what we put into it.
▫️Technically appearing FTB (Not ACB) and comparator FNB, both wont cover required intra-articular innervations I.e. POPLITEAL PLEXUS.
