The #CONCORDStudy may have ended now (though keep an eye out for our @NIHRresearch final report – coming soon!), but there is still plenty of research to be done on care coordination for rare conditions!

Why was #CONCORDStudy needed?
Getting treatment for #RareConditions can be challenging! Rare conditions require care from lots of different sectors, places & professionals. Care may therefore become uncoordinated.
