Today, myself and a GP colleague visited an #agedcare facility for #multidisciplinary #caseconferencing. Think #deprescribing, #collaborative decision making, #patientcentredcare It can be done. What an #onsite #agedcarepharmacist role could look like @PSA_National @RACGP

Pharmacists are an important part of a patient’s care team. These #MBS #caseconferencing items should include them in the interests of quality, safe use of medicines @PSA_National @GregHuntMP @CHFofAustralia

Surely an oversight @GregHuntMP that pharmacists aren’t included as eligible allied health professionals that can be funded to participate in case conferencing - was a recommendation to include pharmacists from MBS taskforce - medicine safety now a #NHPA

#Pharmacists could, if remunerated via #Medicare for #caseconferencing, spend more time in #agedcare facilities working on #qualityuseofmedicines #QUM #NMS2021 @topherfreeman @NPSMedicineWise

"Having #pharmacists on the ground more often in aged care would be a good thing" @topherfreeman reflects on the need for the involvement of pharmacist in #caseconferencing and more #medicinesafety activities inside our aged care facilities #NMS2021
