Risk of interstitial lung disease (ILD) and safety outcomes across #DMARD classes in #rheumatoid #arthritis (RA) and RA-ILD (Korean nationwide cohort study, 26,120 RA patients, 641 with RA-ILD)
◦ ILD incidence was similar across all DMARD classes (anti-TNF, non-anti-TNF biologics, JAK inhibitors, conventional synthetic DMARDs).
◦ No class increased risk of developing ILD or ILD-related hospitalizations.
◦ Non-anti-TNF biologics (abatacept, rituximab, tocilizumab) showed significantly higher risks compared to conventional synthetic DMARDs:
- Hospitalization, especially pneumonia-related (RA: adjusted hazard ratio [aHR] 1.30; RA-ILD: aHR 1.35)
- Serious infections (RA: aHR 1.16; RA-ILD: aHR 1.51)
- All-cause mortality (RA: aHR 4.09; RA-ILD: aHR 4.31)
- Strongest signals in RA-ILD patients, including higher pneumonia recurrence
◦Anti-TNF and JAK inhibitors had safety profiles comparable to conventional synthetic DMARDs (no significant increase in hospitalization, infection, or mortality).
◦ Risk of major adverse cardiovascular events did not differ significantly across DMARD classes.
These findings indicate that while none of the studied DMARD classes appeared to increase the risk of developing or worsening ILD itself, non-TNFi biologics carried the highest burden of serious safety events —particularly infection-related complications and mortality— in both general RA and RA-ILD patients. This study underscores the need for particularly close monitoring for infections (especially pneumonia) when using non-TNFi biologics in these populations.

A recent study shows that the activity of #ACPA-expressing memory B cells is associated with disease phenotype in #rheumatoid #arthritis (RA), may predict imminent RA onset and possibly identify patients in immunological #remission capable of reaching sustained #DMARD-free remission (SFDR). The profile observed in RA (high #CD80, low #CD32, abundant #plasmablasts) is reminiscent of T cell-driven, germinal centre-derived B cell responses, while the phenotype observed in SFDR/nonconverting clinically suspect arthralgia suggests that certain disease features may be absent that otherwise (chronically) stimulate ACPA-expressing B cells. These features may be an important step towards understanding how disease chronicity develops in RA.

A large nationwide study examining all biological and targetet synthetic #DMARD classes, found that #rituximab had the highest incidence rates of major adverse #cardiovascular events (MACE) and numerically increased risk of MACE, as well as the shortest median duration to MACE development, compared with TNF inhibidors exposures, regardless of prevalent cardiovascular disease history. However, this study did not demonstrate a statistically significant higher risk of MACE with either non-TNF and JAK inhibidors exposures compared to TNF inhibidors exposures.
