What’s next for Enhanced Community Care (ECC) in CH East?
Investment in ECC will be delivered on a phased basis with a view to achieving national coverage within 2-3 years. We are currently recruiting for ECC posts and operationalising new pathways of care. #ECCBitesize
The current focus of Enhanced Community Care (ECC) in CH East has 3 pillars. These will work together to provide comprehensive, community-based care close to people’s homes. They will also work to improve the health and wellbeing of the local population they serve. #ECCBitesize
Integration across all Acute Hospital and Community Healthcare services is at the core of Enhanced Community Care. We're looking forward to working with @IEHospitalGroup to develop fully integrated services and end-to-end care pathways. #ECCBitesize
