External Quality Assessment Scheme (#EQAS)—also called External Quality Assessment Services or Proficiency Testing—is that independent check. Blinded samples are sent to participating labs; results are compared with peers and assigned values. The lab then sees how close it is to the truth—not how well it thinks it performed. India’s landmark programme began in 1978 at Christian Medical College, Vellore, under the Association of Clinical Biochemists of India. A pilot had shown alarming inter-laboratory variation. What started with about 50–60 labs grew to nearly 14,000 participants in clinical biochemistry by 2024. Most of those labs process fewer than 100 samples a day—proof that EQAS is not only for large corporate chains. CMC later added haemostasis, transfusion medicine, molecular haematology, microbiology, virology and parasitology schemes. Other NABL-accredited proficiency-testing providers now include RML-QAP, Metropolis EQAS, IAMM, Dr Lal PathLabs, Neu-QAP and MAX QAP. Salient features in India • Voluntary but effectively mandatory for NABL #ISO15189 medical accreditation: at least one EQAS cycle per discipline per year. • #NABL accredits the providers under ISO/IEC 17043; it does not mean CMC or any PT provider “certifies” a lab. Using EQAS for advertising is unethical. • Typical CMC biochemistry cycle: 12 lyophilized human-serum (and urine/HbA1c) samples in three batches a year; online result upload; reports with robust mean, %CV, SDI or Z-score. • Confidential, educational, subsidized and designed for small PHC and district labs as well as teaching hospitals. • Reveals method bias, reagent or instrument drift, and training gaps that internal QC alone can miss. • Over decades, Indian labs have shown falling inter-lab %CV and a rising share of acceptable SDI scores—real quality improvement, not paperwork. EQAS does not replace daily internal quality control. It is the periodic external audit that tells a clinician whether a creatinine, TSH or blood group from one lab can be trusted next to another.
