Dermoscopic findings of the nailfold capillaries and Gottron's papules in dermatomyositis
◦ In most of the cases, #Gottron's papules display pleomorphic vasculature (dotted and linear with branching or linear curved), white scales mostly dispersed unevenly, follicular plugs visible, white or pink structureless regions, and red dots/globules corresponding to punctuate hemorrhages.
◦ Bushy capillaries are the most frequently observed #dermoscopic feature of #dermatomyositis (this is only pattern with a strong association for dermatomyositis over systemic sclerosis), followed by avascular areas.
◦ Although there are no significant difference in disease activity between patients with or without specific nailfold capillary changes, some of these features can improve as disease activity decrease with the treatment.

Inverse #Gottron’s papules are keratotic papules located on the flexor aspects of the fingers (DIP, PIP, and MCP joints), which differentiates them from classic Gottron's papules that occur on extensor aspects. Although Gottron’s papules are are an infrequent manifestation of #dermatomyositis, recognizing this clinical sign can help in the diagnosis of this disorder. In some case series, an association between inverse Gottron’s papules and anti-#MDA5 antibodies or juvenile dermatomyositis has been reported, and patients with this clinical sign might have an increased risk of developing interstitial lung disease, as well as a worse prognosis.

Skin ulcerations associated with #MDA5-positive #dermatomyositis (DM) usually manifest as deep painful ulcers localized over #Gottron's papule, involving the digital pulp and nail folds, or over Gottron's sign on the knees, elbows, or both. Oral ulcers are also more frequent in this subtype of DM. Patients with ulcerative Gottron’s papules are more likely to test positive for anti-MDA5 antibody and are at great risk for developing interstitial lung disease.
