Having attended the #HSIBcon22 today (which was great!) it got me thinking...! So, working on the premise that all models are wrong, but some can be useful, this is what I've come up with to help my thinking about further developing an 'engaged safety culture' in health & care.
A big shout out to everyone who has contributed to making #HSIBCon22 such a great learning experience today – our speakers, guests and all of the dedicated healthcare staff who joined us to talk #PatientSafety.

#HSIBCon22 Very concerned to hear that an HSIB spokesperson is recommending extension of #safespace to maternity investigations and even local PSIRF investigations.This would drive a coach and horses through the #dutyofcandour which is one of the reasons it was soundly rejected

Great session with Paul Bowie and Clare Crowley on assessing investigation reports. Lots of traps to avoid- will think more on counter factual reasoning and lack of rationality. Good to see avoidance of weak recommendations and safety clutter. #HSIBCon22 @hsib_org
