◦ X-ray shows a pattern of #calcification of #renal parenchyma compatible with medullary #nephrocalcinosis, being dRTA the most common cause of nephrocalcinosis due to #hypercalciuria without hypercalcemia.
◦ dRTA is a rare but serious type of kidney disease characterized by an impairment in hydrogen ion secretion in the distal tubule, resulting in a persistently high urine pH (> 5.5) and systemic acidosis.
◦ It can be inherited (primary dRTA, also known as Albright acidosis) or be caused by another disorder or medication (secondary dRTA).
◦ Renal manifestations classified as renal tubular acidosis usually occur in a variable proportion, ranging from 3.6% to 73.1% in series of patients with Sjögren's disease. Other causes of secondary dRTA include systemic lupus erythematosus, kidney transplant, medullary sponge kidney, chronic obstructive uropathy, drugs (amphotericin B, foscarnet, lithium), cirrhosis, sickle cell anemia.
◦ Hypercalciuria in dRTA is often multifactorial (bone calcium release, calcium-sensing receptor suppression, increased distal sodium load, and acidosis), which, together with alkaline urine and hypocitraturia, favors #lithiasis and nephrocalcinosis.
