18th July @guguddetvuganda discussed #LabInvestigations with @jcrc_official1 Specialist Mr Benson.
It is not ethical to detect to the clinician the tests you wish to be done. Clinicians are trained and are competent to assess your situations and request for tests he/she thinks will be beneficial to your health. But today due to the Internet and social media, each is a clinician in his/her own wisdom.
This may have dare consequences in terms of cost, creating anxiety, and many other negative effects. Because you may fail to help yourself after receiving the negative or unanticipated results.

Laboratory investigations are relevant because:
Each patient, researcher, traveller, or a Doctor, desire to have a quality result.
A quality result must have the following attributes:
Reproducible, reliable, and timely
If any of the above falls short, then that result has not met the purpose
The list of lab tests done at JCRC is many.
We have several departs in the lab, and each depart has specialized tests using STATE-of-ART Equipment.
We also have BSL 3 labs to handle any infectious disease that may occur in the country
One of these labs we use it for TB tests Cultures. Other TB tests are (microscopy, GeneXpert). We also do HPV screening to rule of cervical cancer in women
Chemistry: LFT (Albumen, T. Protein, bilirubin, ALT, AST ALP etc); RFT (Urea, electrolytes, Creatinine etc); Glucose, Glycerated haemoglobin (HBA1C); and PSA in men; Automated Hepatitis B and C screening, Toxoplasma, CMV etc
Because we are starting BMT, we are introducing tests for monitoring certain drugs in the patient; Cyclosporin and tacrolimus.
Hematology: CBC, Malaria, ESR, Sickle cell genotype; Coagulation tests (Thrombin time, prothrombin time, INR, Factors VII & IX etc); ABO blood grouping & Screen for antibodies, and Cross match.
Immunology: HIV screening, HIV confirmatory tests, CD4/CD8, TB QuantiFERON, and other research tests
Microbiology: Blood culture for bacteria and fungi, swabs collected from different body parts, urine culture and urinalysis, microscopy, Syphilis testing, etc.
Molecular Biology:
HIV RNA viral loads, Hepatitis B & C viral loads, STI (Neisseria gonorrhoea, Trichomonas vaginalis, Chlamydia trachomatis)
HIV Drug resistance testing. Other research assays.
Indications for lab tests: This is usually determined by the clinician who has access to the patient. He/she interacts with the patients and comes up with a request form for lab tests or any other tests outside the lab.
How do you maintain quality?
This is a very expensive investment that an institution that cherishes the quality of the lab results will have to be undertaken. Is not an option!
In the lab, we have a quality management system (QMS) built on 12 pillars, but I will mention a few;
Staff, Infrastructure, Quality Equipment & validate or verified, Good supply of reagents/controls, External Quality Assurance scheme(s), Internal audits, Safety of staff and surroundings, Internal controls, Lab Information management system (LIMS)
JCRC Laboratories (Lubowa) are accredited by the USA College of American pathologists (CAP) since 2017 to date, with inspections after every 2 years
JCRC Laboratory in Mbarara Hospital is accredited by the South Africa national Accreditation Scheme (SANAS).
This FY, we are embarking on preparing Fort Portal Lab for accreditation, then Gulu like that…
ii. diagnosis?
Accurate diagnosis heavily relies on lab tests and other diagnostic tools.
But this journey has yielded good results, and now Uganda lab is divided into 3 main components:
Pre-examination: phlebotomy, collections of samples, account for over 70% of the diagnosis of a patient. So, lack of or poor lab results may lead to poor patient outcomes from his/her sickness.
