As an editor, I often read discussions that are long winded, meandering and fail to make an impact.
They feel like an after-thought rather than a key part of the paper.

How do we improve the transparency of randomized controlled trials? One important method is the publication of the protocol prior to the trial. Entire journals are focused on this.
