Leukocyte Cell–Derived Chemotaxin 2 (#LECT2) #amyloidosis is a rare form of systemic amyloidosis with predominant involvement of #kidney and #liver. It is characterized by deposits of LECT2, a protein made in hepatocytes and released to circulation with multiple biologic functions, including liver regeneration and fibrogenesis. The disease has a strong ethnic bias, affecting mainly Hispanics (particularly Mexicans) and a few other populations in the world, including Punjabis and Egyptians.
Most patients are elderly who present with chronic renal insufficiency, variable proteinuria and bland urinary sediment. Liver involvement is frequently an incidental finding, but it can cause cholestatic jaundice, vanishing bile ducts, nodular regenerative hyperplasia, and idiopathic noncirrhotic portal hypertension. LECT2 amyloid deposits shows a characteristic distribution: in the kidney, there is consistent involvement of cortical interstitium, whereas in the liver, there is a preferential involvement of periportal and pericentral vein regions.
There is currently no efficacious therapy for LECT2 amyloidosis. However, patient survival is excellent, likely because of the rarity of cardiac involvement. Renal transplantation seems to be a reasonable treatment for patients with advanced renal failure, although the disease may recur in the allograft.
