#Microgeodic disease is a rare primary #osteolysis disorder characterized by multiple small #osteolytic areas and sclerosis in the phalanges of one or both hands associated with swelling and redness of the phalanges. The term microgeodic was coined by a French radiologist Maroteaux to describe the X-ray finding of very small (< 1mm) punched out lacunar-like osteolytic lesions in the small bones of the fingers of infants affected by this condition. The average age at diagnosis is 7 years. Microgeodic disease usually occurs in the winter months. Patients present with swelling, redness, and sometimes pain in one or more fingers or toes (hands are more commonly affected than feet). Often the clinical changes are described as 'chilblain-like'. The disease course is mostly benign, however, complications such as fracture and digit shortening can occur. While the pathogenesis of this disease is unknown, it is thought to be a transient disturbance of the peripheral circulation caused by cold temperatures. A spectrum of disease with #Raynaud’s and #chilblains at the milder end, and microgeodic disease at the more severe end has been proposed. The differential diagnoses include sickle cell dactylitis, chilblains, spondyloarthritis or infections including cellulitis, osteomyelitis, syphilis and tuberculous dactylitis.
