My 10 points:
1. Scapula is complexly innervated Structure. "Identify-Select-Combine" Approach helps to plan RA technique for any such complexly innervated structures.
2. Most of its innervations are derived from C5 & C6 roots. However, all innervations are derived from cervical and brachial plexus EXCEPT skin of back region.
3. Skin over scapula is supplied by Dorsal Rami (T2-T9/10). For this ESP with Diluted LA will be sufficient.
4. Muscles attached to the scapula are innervated by Brachial Plexus.
5. Understanding Steps of Sx, Positioning of Patient, Duration of Surgery, Incisions,& Innervations of involved structure is essential to plan Suitable RA technique.
6. Planned procedure-Specific RA technique should cover innervation of incisional area (Dermatome) & involved scapula bone (Osteotome).
7. Myotomal coverage needed only for the muscles which are involved in surgical dissection.
8. Myotomal coverage for muscle relaxation is needed only if Surgery will be under Sole RA. For that high conc. LA can be used.
9. For analgesia purpose, Myotomal coverage is not needed. So diluted LA can be used (Motor-Sparing block).
10. Depending upon the surgical plan and extending dissections, Combinations of blocks are required for Scapula surgery. For that knowledge of Spared innervations of primary blocks and knowledge of Supplementary blocks to cover those are essential.
In our Case, We gave Combination of
SCUT (SupraClavicular + UT) block to involve C5 & C6 + SCN (to cover skin over & above spine of scapula).
SAP to cover Axillary dissection (T2-T9) with LTN & TDN.
ESP to cover skin over the Scapula.

It can be used as a Sole RA technique
OR
For Analgesia purpose in pt under GA+RA
Better results if given preoperatively >20-30 min before surgical incision.
