Tuberculous cervical #lymphadenitis, also known as #scrofula, is the most common presentation of extrapulmonary #tuberculosis, especially in pediatric patients, and it is currently treated with a four-drug empiric regimen of isoniazid, rifampin, pyrazinamide, and ethambutol. However, until the 18th century this disease was treated in a rather peculiar way, from which its old name derives: “King’s-Evil”, because in Europe it was thought to be cured by the royal touch. The first reference to this type of royal "healing" practice dates back to William of Malmesbury (ca. 1090-ca. 1143), who described the use of the thaumaturgic touch by King Edward the Confessor (1003-1066) in a woman afflicted with scrofula. While this power to heal the scrofulous was initially ascribed to King Edward's 'saintliness', it later became politically expedient to link the performance of miracle to the dynastic lineage in order to legitimize the right to rule. While the ritual changed significantly over the centuries, it often consisted of Scripture readings, prayers, the placement of a gold coin around the neck of the ill, and –most importantly– the imposition of the monarch's hands on the ill as the central curative act. This ritual was legitimized in borrowing a custom from the Franks whereby the English monarchs were crowned and anointed with oil by a bishop, most notably on their hands: this placed the monarch in the same rhetorical space as priests, allowing them to claim a position as mediator between God and subject, further entrenching the legitimacy of their rule and allowing them the power to heal. Within this rhetoric, the body politics was conceived in corporeal terms. The monarch was its head: the subjects, the remainder of the body. I learned this recently in the context of a patient with AIDS, who had tuberculosis exposure, and a mass of necrotic lymph nodes in her neck that looked exactly like every picture of #scrofula that you can find. I held out for six weeks until the cultures were final.

@BrownHospMed TB adenitis #scrofula. More common I. Kids and women vs pulmonary TB - which is more common in me. Could be multi drug resistant due to presence of #HIV. Get sensitivities but in meantime ask ID what to start with.

@jacquep @jgraddon He is lying. On News it confirmed he’d visited a London hospital where there were Covid-19 patients, but he didn’t shake hands with any of them. Not clear that he went anywhere near them.
