With the great results of #SELUTION4ISR & #SELUTION DeNovo trials at #TCT2025, the "lowly balloon" is being reborn for PCI. Since I'm always thinking about #AngioHistory, I thought this clip from my 1985 (i.e. pre-stent) interview w Andreas Gruentzig would be appropriate today.
Results from #SELUTION are encouraging, but some aspects deserve attention: 👇
1️⃣ Observed TVF rate of 4.4% (DES) was lower than expected (assumed rate 6%)
2️⃣ 50% non-inferiority margin is somewhat large. With a more conservative approach, including a 33% margin, non-inferiority wouldn’t hold (Upper CI 2.38% higher than 1.60%)
3️⃣ Periprocedural MI ≈1%: very low, raising possible underreporting.
Overall, this is a technology with a great potential, but let’s safeguard it through trial evaluation and critical scrutiny, not overstatement. We should embrace innovation with hope and thoughtful assessment, not hype.

Large, pragmatic strategy trial: sirolimus DCB-first vs contemporary DES for de novo lesions.
Unique strategy: patients randomized before lesion preparation — unlike prior DCB studies.
✅ Non-inferior for 1-year TVF (5.3% vs 4.4%)
💀 Death, MI, thrombosis all similar
🧠 80% of DCB patients avoided a stent
⚠️ Slightly higher TVR (3.3% vs 2.1%)
🔍 Low intravascular imaging use (~16%) outcomes could be even stronger with optimized prep & imaging guidance.
A real step toward “leave nothing behind.”

@mmamas1973 We don’t want to leave metal behind however the early POBA experience was marred by abrupt vessel closure..back to that practice requires evidence #SELUTION De Novo is certainly an important step in that direction
