Our next #TipsforTuesday is from Dr. Zhang, who shares his technique for Arthroscopic SLAP Repair with Percutaneous Suture Passage Through the Neviaser Portal.
“After placement of an all-suture anchor on the glenoid in the posterior aspect of the biceps anchor, a spinal needle is used to pass a PDS suture through the base of the SLAP tear percutaneously through the soft spot of the Neviaser portal. The tip of the spinal needle should be visualized intra-articularly first before passing it behind the labrum as this will help with suture retrieval. The PDS suture is then passed through the spinal needle and used to shuttle the suture of the labral repair anchor out from the Neviaser portal. The sutures are then retrieved and tied through the anterior portal making sure to not strangulate the biceps tendon. These steps are then repeated with an anchor anterior to the biceps anchor to complete the SLAP repair.”

When preparing osteochondral allografts, Dr. @jrodsportsMD recommends drilling several 1.0-millimeter K-wire fenestrations to facilitate creeping substitution.
Afterward, clean the graft of marrow elements using pulse lavage and compressed CO₂. Once dry, the graft readily soaks up the patient’s prepared BMAC when applied with a syringe – leaving it ready for implantation.

Consider fixating the biceps tendon prior to release. This can be done with an arthroscopic 2-anchor onlay technique within the groove of the biceps:
Steps:
1️��� Expose the biceps through the rotator interval (see prior Tips for Tuesday)
2️⃣ Place a traction stitch around the biceps tendon and prepare the bony bed
3️⃣ Place 2 suture anchors inside the groove to create 2 traction stitch patterns through the biceps tendon
4️⃣ After the biceps has been tenodesed to the bone within the groove, THEN perform the proximal tenotomy and remove any excess proximal biceps tissue
5️⃣ You may or may not repair the rotator interval over the top of the biceps tenodesis or incorporate this into an adjacent rotator cuff repair
Utilizing these steps will set the anatomic tension of the biceps every time before the biceps is released from the superior labrum – no more guesswork to establish tension!

“After placing your guide pin in Schottle’s point for MPFL reconstruction, consider shuttling the sutures from the most distal anchor point on the patella between layer 2 and 3 and wrap around the guide pin to evaluate the isometry of our pin placement. If the suture tightens in flexion, move the pin distal. Conversely, if the graft tightens in extension, adjust the pin anteriorly to balance your isometry. Checking isometry prior to drilling the femoral tunnel and cutting the graft will ensure a successful and balanced MPFL reconstruction!”
