#XComment 💬 Rethinking BSO Timing for BRCA Carriers in the Era of PARP Inhibitors Bilateral salpingo-oophorectomy (BSO) remains a cornerstone of cancer prevention for BRCA1/2 mutation carriers. Extensive literature shows that BSO significantly reduces the risk of ovarian and fallopian tube cancers, as well as overall mortality, making it a critical strategy for this high-risk population. But with the emergence of PARP inhibitors like olaparib—now approved as adjuvant therapy for early-stage, high-risk BRCA-mutated breast cancer—could we reconsider the optimal timing of this surgery? The OlympiA trial, with a median follow-up of 6.1 years, showed that one year of adjuvant olaparib improved invasive disease-free survival (HR 0.65) and overall survival (HR 0.72), and reduced the incidence of second primary cancers: 3 ovarian cancers (vs. 9 with placebo) and 1 fallopian tube cancer (vs. 4). Although not administered for prophylaxis, these findings suggest that adjuvant olaparib may confer ancillary risk-reducing effects, potentially reshaping surgical decision-making for some patients. For premenopausal women—especially those prioritizing fertility or concerned about the impact of early menopause on quality of life—could such systemic therapies support a safe deferral of BSO? Current guidelines recommend BSO between ages 35–40 for BRCA1 carriers and 40–45 for BRCA2 carriers, based on legacy risk data that predate modern therapies. While surveillance with transvaginal ultrasound and CA-125 is standard for those delaying BSO, these tools might also serve patients receiving adjuvant PARP inhibitors, offering a framework for structured monitoring during a period of intentional deferral. The possibility of PARP inhibitors serving as a “bridge” strategy—providing temporary protection while aligning surgery with life plans—remains underexplored in clinical guidance. Given BSO’s irreversible effects on fertility, hormonal health, and overall well-being, a more personalized approach is essential. Integrating systemic treatments like adjuvant PARP inhibitors into the timing discussion could empower patients and clinicians to make decisions that reflect both cancer risk and individual goals. Could PARP inhibitors help redefine cancer risk management for BRCA carriers? Let’s discuss what personalized care might look like in this evolving landscape🩺🧬 Yakup Ergun, MD #BRCA #CancerPrevention #bcsm #PersonalizedMedicine #Oncology

#XComment on DYNAMIC trial ctDNA-Based Approach in T4 Tumors When it comes to T4 stage II colon cancer tumors, caution is essential before adopting a ctDNA-based approach. While the DYNAMIC trial suggests potential benefits, several critical limitations raise doubts about its reliability specifically for T4 patients. Below, I’ll break down these concerns in a straightforward way to highlight why we need to tread carefully. Small Group Size and Unreliable Stats: The study only looked at 64 T4 patients—44 in the ctDNA-guided group and 20 in the standard group—out of 441 total participants. Survival numbers look promising (90.5% vs. 80% for OS; 81.2% vs. 70% for RFS), but the stats don’t hold up strongly. The wide ranges (e.g., 0.53-8.79 for survival) and lack of clear significance (P-values of 0.27 and 0.28) mean the small sample size raises the risk of a type II error (failing to detect a true difference). This limited statistical power makes the findings uncertain. Unknown MSI Details Messing with Treatment: We know some T4 patients have MSI, with about 12-13 of the 64 likely having this trait (based on 19.5% of the whole study). For those with pMMR, survival looks better with ctDNA guidance (87.1% vs. 77.8%), but we’re in the dark about the MSI-high/dMMR group. The problem? Most standard T4 patients got 5FU alone (12 of 14), which doesn’t work well for MSI-high tumors. Meanwhile, ctDNA-guided patients got more oxaliplatin (7 of 12), which might help, but we don’t know how MSI-high T4 fares specifically. This gap makes it hard to trust the approach for all T4 cases. Mixed Treatments Clouding the Picture: In the ctDNA-guided T4 group, only 12 of 44 got chemo—7 with oxaliplatin, 5 with 5FU alone. In the standard group, 14 of 20 got chemo, but 12 got just 5FU, and only 2 got oxaliplatin. This mix-up makes it tricky to tell if better outcomes come from ctDNA or just using stronger drugs like oxaliplatin (which clears ctDNA better: 92.3% vs. 78.6%). Without sorting this out, we can’t pin the success on ctDNA alone. Missing Recurrences in “Safe” Patients: For T4 patients testing ctDNA-negative (supposedly low risk), the recurrence rate is worrying—4 out of 5 recurrences were local, and their RFS (81.3%) lags behind T3’s (91.7%). The test only catches about half the residual disease, so in aggressive T4 tumors, skipping chemo based on a negative result could leave cancer lurking. This risk of undertreatment is a big red flag. Conclusion: For T4 tumors, these issues—too few patients, unclear MSI effects, jumbled treatments, and missed recurrences—mean a ctDNA-based approach isn’t ready to fully trust yet. It shows promise, but we need more data to be sure it’s safe and effective for this high-risk group. https://t.co/5wXyaTEl9I
