Innovative and sustainable financing is the backbone of strong primary health care (PHC). Social health insurance (SHI) is emerging as a game changer in the journey toward Universal Health Coverage. By pooling risks, expanding coverage, and mobilising domestic resources, SHI can shift countries from fragmented, out-of-pocket spending to equitable, predictable, and people-centred health financing systems. Success depends on smart design, including the inclusion of informal-sector populations, strategic purchasing, accountability, and alignment with national health priorities. As countries face rising health demands, fiscal constraints, and evolving disease burdens, the question is no longer whether to adopt SHI but how to make it efficient, inclusive, and sustainable at scale.
