In May 1911, American neurologist and psychiatrist Charles Gilbert #Chaddock (1861–1936), translator and admirer of the work of Joseph #Babiński, presented at the St. Louis Neurological Society the preliminary communication of his modification of the Babiński sign. Chaddock described that stimulation of the lateral surface of the foot (S1 dermatome) could also induce dorsiflexion of the hallux and fanning of the other toes in patients with pyramidal tract lesions. The most sensitive area for eliciting his external ankle sign as overlying a slight depression just in front of the lowest point of the external malleolus and behind the tuberosity of the cuboid. He believed his sign to have the significant advantage of minimising dorsiflexion of the foot as a withdrawal reaction to noxious stimulation: “I have found the extension of one or more or all of the toes with or without fanning of them when the external inframalleolar skin is irritated in cases of organic disease is a spinal cortical reflex path. I shall call it the ‘external malleolar sign’ external malleolar sign.” A study published in 2015 that compared the Babiński, Chaddock, and Oppenheim reflexes found that the Babiński was of higher sensitivity, and that the Babiński and the Chaddock demonstrated similar positive predictive values (70.3% and 66.5%, respectively). Like the Babiński reflex, the Chaddock reflex may also be positive in healthy babies until 2 years old because infants have an incompletely myelinated corticospinal tract.
