#Lipodermatosclerosis (LDS) is a progressive manifestation of chronic venous insufficiency characterized by distinctive lower extremity changes, including an “inverted champagne bottle” appearance, skin hyperpigmentation, and occasionally, non-healing ulcers. Despite LDS has not been extensively studied in the context of connective tissue diseases, it shares similar pathogenic pathways with systemic sclerosis (SSc), where vasculopathy leads to tissue hypoxia, inflammation, and subsequent fibrosis of the skin and subcutaneous tissues.
In this study that included 567 patients with SSc, 25 (4.4%) had LDS and ulceration was observed in 8 of them. Cardiac arrhythmia (45.8% vs 26.9%), heart failure (28.0% vs 10.1%), and pulmonary hypertension (48.0% vs 25.3%) were more common in patients with LDS vs those without LDS, respectively. These findings indicate that clinicians should recognize the features of LDS in SSc patients and closely monitor these patients for pulmonary hypertension and other macrovascular complications.
