24a) A 32♀️ desiring pregnancy had very symptomatic #oHCM, meeting diagnostic criteria even without #provocation. She was symptomatic despite #betablocker therapy.
17b) Because mortality rates are lower in #oHCM trials than in #HF trials, endpoints are "softer" (recall this discussion is at #HeartFailure2025!)--but they are predictable, consistent, & associated w/substantially ⬇️ symptom burden that corresponds to ⬇️ of #LVOT gradient.
17a) Now we have #CMIs: the approved #mavacamten & the still investigational #aficamten. Mavacamten currently has a Class I indication in #oHCM, with DURABLE clinical effects largely on par with surgery. Pts should now be offered the option early: 💊 or 🔪
16) In fact this has spurred advances in #oHCM management beyond the often limited effects of #betablockers and #disopyramide. Invasive #SRT was really the only effective strategy for oHCM.
