❌💊DDI caught by pharmacist! Patient admitted in VT and started on amiodarone. Pt also had AML on venetoclax which was restarted by MD. Significant DDI with amio and venetoclax w/ increased venetoclax levels when given with P-gp inhibitor. #pharmessentials #pharmsaves #PharmICU

Here's a Good Catch! ☠️ Patient admitted w/ sepsis, AKI, & hyperkalemia. Pharmacist reviewed home medications & noted patient was on digoxin. There was concern for digoxin toxicity based on labs & they recommended obtaining a digoxin level. #pharmessentials #pharmsaves #pharmICU

The pharmacist suggested possible malignant hyperthermia as case reports describe rare occurrences with non-depolarizing NMBAs. Recommended checking CK and initiating dantrolene. Immediately after dantrolene, hyperthermia resolved! 🙌 #pharmessentials #pharmsaves #PharmICU

🔥 A rare case of malignant hyperthermia was promptly identified & treated by a pharmacist! A patient with severe PNA requiring cisatracurium developed Tmax 41°C, tachypnea, & respiratory acidosis that failed to respond to external cooling. #pharmessentials #pharmsaves #PharmICU

5 min after receiving neostigimine patient became diaphoretic, nauseous & felt the sensation of throat closing. RRT called + ICU pharmacist contacted and recommended prompt treatment with IV glycopyrrolate with prompt resolution of symptoms! #pharmessentials #pharmsaves #PharmICU

🙌 Good catch of the month! 🙌 The ICU pharmacist recommended prompt treatment for a suspected neostigimine reaction. Patient with multivessel CAD s/p CABG; post-op course was complicated by ileus & patient received neostigimine on POD7. #pharmessentials #pharmsaves #PharmICU
