Are we sure #PTMV should really be done today according ancient device and technique? We can apply what we learned from other structural interventions: #echofirst transeptal puncture, guidewire-guided balloon advancement and pacing..
https://t.co/Q6th8IRty0
The problem :
The INOUE balloon has 2 layers, the outer latex layer of Balloon was probably cracked or cut partially at the WAIST , which was not recognised while preparing it may be , with wach inflation the cut may have become deeper and then outer layered ruptured completely at the end causing it to form OVAL SHAPED rather than DUMBBELL ..
Solution : New balloon..
Patient did well ..
@Hragy may be u can add some pointers to overcome it , although i know that U alresdy told that REUSE has been banned in EGYPT ..

That’ really great! Thx for sharing. The best award for a new technique is that it is able to help different interventionalists!!! #PTMV.
@CristinaAuri17 @Carlotrani @BURZOTTA_F Following your publication and thanks to your advice, I successfully used your technique in my patient: inflation with 24 and 26 balloon and we achieved reduction of mean gradient from 6.7 to 3 mmHg and increase of MVA from 0.9 to 1.5 cm2. Thank you very much!
Not happy with Inoue #PTMV ?? Just published in @EuroInterventio: our NEW DIRECTIONAL TECHNIQUE for percutaneous #mitralvalve balloon #valvuloplasty. Enjoy the beauty of using a similar approach for all #heartvalves dilation. Great #teamwork in our #cath lab! #structuralheart
