We had a lively discussion about GLP1RA (e.g. #semaglutide #tirzepatide #dulaglutide #Ozempic #Mounjaro #Trulicity) on our journal club last Tuesday which gave me the opportunity to share some thoughts about these agents let's follow with a couple of papers.
The perspective I have taken is to use these drugs(*) as if I am managing weight in primary care but adding the twist that the individuals with #kidneydisease #CKD I am treating are at high risk of intolerance and other side effects.
Which resources am I using to support this practice ?
First there is a nice PCP oriented paper with info graphics written from the UK perspective (this is important because they tend to be less aggressive and more practical when titrating medications there)

The paper briefly touches the concept of micro dosing (a practice historically associated with weight loss cash only clinics in the US that were using compounded meds).
This brings us to the second paper in Diabetes Care discussing how one can do microdosing practically with the multi dose pens. Some of the patients with #CKD may tolerate this approach better so may want to know about this and how to do it

One of the issues that comes up with these drugs are visual problems: #GLP1RA have had warnings about worsening Diabetic retinopathy but there are concerns about a non diabetic retinopathy based on recent observational data.
The transient worsening of retinopathy with improvement on glycemia is not specific to GLP1RA and has been with other interventions including insulin therapies (this is a nice somewhat date review but discusses this)

I am going to nerd out now and bring up the point that hypoglycemia likely mediates the effect through HIF1

so *perhaps* once we practically get HIF1 inhibitors for anemia management we will have a way to counteract the effect, but for now avoid hypoglycemia.
With regards to the second retinal side effect there is a 20 year old paper at PNAS that looked at hypoglycemia and age related eye loss that is mind blowing

Since patients with #CKD are more likely to develop hypoglycemia esp as kidney function worsens, it is important to prevent that, hence slow titration/microdosing/and even continuous glucose monitoring with one's patients.
