The ending makes sense when you realize they both got exactly what they wished for. Bear wanted Nikki to love him above everyone else, and Nikki wished for Bear to love her just as intensely. When they smile, it's because, for one brief moment in the scene, both of their wishes finally come true.

I've never seen a movie ending that matched its story as perfectly as this one. #Obsession
