Why?
ALT (Alanine aminotransferase) is the most sensitive and specific marker of hepatocellular injury.
It is located predominantly in hepatocytes, so even mild liver cell damage causes ALT to rise.
Why not the others?
ALP → marker of cholestasis (bile duct obstruction), not primary hepatocyte injury
GGT → sensitive to cholestasis and alcohol use, but not specific for hepatocellular damage
Bilirubin → reflects excretory function, rises later and is not sensitive for early injury

> Hepatocellular injury → ALT (± AST)
Cholestatic injury → ALP + GGT
So, the most sensitive marker of hepatocellular injury is ALT.
