@MGalganoMD @AJGrossbach @NaderDahdaleh @jsteinbergerMD If there was already chest drain w water seal and diligent pleural repair and multilayer dura repair done, I'd try LD and positive pressure ventilation first and be patient to reduce the negative intrathoracic pressure. Repeat repairs wld hv diminishing return bt may be necessary

I would probably do mini open lateral retropleural approach (with the "egg beater retractor") for discectomy and MIS (robotic assisted) posterior instrumentation in lateral position. Dissection off the duration will be challenging.
