Up until about week 7 to week 8 of pregnancy, both sexes have what’s known as a “genital ridge” — i.e., an identical preliminary set of genitalia that will eventually differentiate to become either male or female sex organs.

That means that our sex organs come from the same foundations: The testes in men are equivalent to labia and ovaries in women, and the penis is the equivalent of the clitoris.

They inject the opposite hormone (a blood test will reveal if there's testosterone presence in the mother's blood) into the womb.

The result is that the external part of the body is formed according to the hormone being injected. So A boy will be born, with the skeleton of a woman, with a penis, but he'll have to take external testosterone for the rest of his life.
Conversely, if estrogen is injected, the boy will be born with a vagina but will not be able to have kids, since they cannot pass through the narrow cavity in the pelvis.
