WHAT THE DRUGS MAY DO
• Fenbendazole/mebendazole: bind tubulin, collapse microtubules, cause mitotic arrest, move p53 toward mitochondria, and suppress GLUT transporters and hexokinase II.
• Ivermectin: studied for PAK1, WNT/beta-catenin, P-glycoprotein, mitochondrial complex I, and KPNB1 nuclear transport.
• Niclosamide: mitochondrial uncoupler affecting STAT3, WNT/beta-catenin, NF-kB, mTORC1, and Notch.
• Artemisinin: iron cleaves its endoperoxide bridge, releasing free radicals; cancer cells may carry more transferrin receptors and free iron.
• Atovaquone: inhibits mitochondrial complex III, potentially lowering tumor oxygen use and hypoxia while improving radiation response.
• Itraconazole: blocks Hedgehog signaling at Smoothened separately from killing fungus; may also affect VEGFR2, angiogenesis, mTOR, and cholesterol trafficking.
• Ketoconazole: inhibits CYP17A1 and androgen synthesis, explaining its historical use in prostate cancer.
• Griseofulvin: binds tubulin and shares a mechanism family with benzimidazoles.
Fungi, parasites, and cancer cells are eukaryotes. They share tubulin, mitochondria, nuclear pores, and glucose transporters. A drug aimed at a rapidly dividing eukaryotic cell may affect both a worm and a tumor. Benzimidazoles target the same general machinery as vincristine and paclitaxel.
IS THERE FUNGUS IN TUMORS?
Two 2022 Cell studies examined 35 cancer types. One analyzed 17,401 samples across four cohorts and found cancer-type-specific fungal patterns. Another reported up to about one fungal cell per 10,000 tumor cells, with Candida enriched in gastrointestinal sites and associated with worse survival.
In pancreatic cancer, researchers reported fungal loads about 3,000 times higher than in healthy pancreas, with Malassezia enriched. The proposed mechanism involved mannose-binding lectin and complement activation. Antifungals slowed tumors in the model; reintroducing Malassezia accelerated them.
This does not prove all cancer is fungal. A foundational 2020 Nature microbiome paper was retracted in June 2024 over insufficient filtering of human sequences, organisms not known in humans, and processing artifacts. Later studies excluded 95 fungal species as likely contaminants, but low-abundance findings remain vulnerable to reagents, paraffin blocks, equipment, and processing.
Four observations challenge a universal fungal-cancer theory:
1. Cancer does not generally transmit between people like fungal disease.
2. Transplant recipients and people with HIV have much more invasive fungal disease, but not every cancer; many excess cancers are viral.
3. Decades of antifungal use have not broadly reduced cancer incidence.
4. The clearest established fungal contribution is aflatoxin, a toxin that mutates p53.
THE HONEST VERDICT
Fungi may be residents or modulators of some tumors, shaping inflammation, immune tone, and outcomes. That supports a passenger-and-accomplice model in selected cancers, not a universal driver theory. When itraconazole affects basal cell carcinoma, Hedgehog blockade is the relevant mechanism, the same pathway targeted by vismodegib.
For any organism-cancer claim, ask whether the result survives a change in method, whether cases and controls were handled identically, whether exposure tracks disease across time and geography, and whether removing the organism changes the disease. H. pylori passes the last test because eradication can place about 60% to 80% of early gastric MALT lymphomas into remission.
These drugs deserve research based on their real mechanisms, including potential tubulin toxicity, interactions, liver-enzyme concerns, and monitoring needs. Tell your oncology team everything you use.
