Gender aligns with birth biology. Intersex (khuntha) cases allow determination of the predominant sex, sometimes with corrective surgery. HRT or surgery for gender dysphoria is prohibited as altering Allah’s creation (4:119) and imitating the opposite sex (cursed in Hadith). It does not change one’s legal sex under Sharia.
