For those jumping straight to “BMV” , yes, that may be correct, but first learn to read the cath tracing before naming the diagnosis.
Interpreting cath tracings is like reading an arrhythmia ECG:
go stepwise, don’t jump to conclusions.

What is this tracing?
👉 Simultaneous LV and LA pressure tracing

How do we know it’s LV (and not RV)?
• LV waveform is squarish
• RV waveform is triangular
• LV tracing here is not properly zeroed ; note LV diastolic pressure dipping below zero

How do we know it’s LA (and not RA)?
• LA has a prominent V wave
• RA typically has a dominant A wave
• A wave is identified by temporal correlation with the P wave on ECG

Key hemodynamic findings:
• Persistent gradient between LV diastolic pressure and post-A wave LA pressure → suggests LV–LA gradient
• Large V wave occurring during LV isovolumetric contraction → indicates mitral regurgitation

Take-home:
This tracing is most consistent with a post-PTMC (BMV) hemodynamic pattern with residual MR.

My point being —> Read the waveform first. Diagnose later.
