Gitelman syndrome (GS) is a rare, autosomal-recessive salt-losing tubulopathy often requiring lifelong electrolyte supplementation. A new case report describes a patient with GS who initiated tacrolimus, a calcineurin phosphatase inhibitor, after orthotopic heart transplantation.
