🚨 Heart disease is not inevitable. We have the tools right now to prevent the majority of cardiovascular events before they happen. The question is no longer whether we can eliminate heart disease. The question is whether you will act before your first event or after.
Most people wait for the chest pain. That is the wrong strategy.
💓 Here is what the science actually says.
Cardiovascular disease kills more Americans than any other condition. But the biology of atherosclerosis gives us a long window to intervene. Plaque does not build overnight. It builds over decades. And that means we have decades to stop it.
🔬 The mechanisms are clear:
LDL drives plaque accumulation. Lower it early, lower it aggressively, and you reduce events.
ApoB is the more precise target. Every atherogenic particle has one ApoB molecule. High ApoB with a normal LDL still kills people.
Lp(a) is inherited, largely unmodifiable today, but knowing your number changes how aggressively we treat everything else.
Inflammation amplifies the damage. CRP, hsCRP, IL-6. These are not just markers. They are active contributors.
✅ JUPITER (rosuvastatin): cardiovascular events reduced 44% in people with elevated hsCRP and normal LDL. Normal LDL. Still at risk. Still responded to treatment.
✅ FOURIER (evolocumab): major adverse cardiovascular events reduced 15% on top of maximally tolerated statin therapy. LDL driven below 30 mg/dL in many patients with no safety signal.
✅ ORION-10 (inclisiran): LDL reduced 52% with two injections per year. Adherence solved. Two shots. Twelve months. Done.
✅ SELECT (semaglutide): major cardiovascular events reduced 20% in patients with cardiovascular disease and overweight, without diabetes. Weight loss is now a cardiac intervention.
🩺 I am a board-certified cardiologist. I see patients every week who come to me after their first heart attack asking what they could have done. The answer is almost always: this. Earlier. We could have done this earlier.
A patient who knows their ApoB, their Lp(a), their hsCRP, and their blood pressure at age 40 can build a prevention strategy that takes their lifetime risk from catastrophic to manageable in five to ten years. That is the difference between a stent at 58 and a clean stress test at 68.
⚠️ What is standing in the way is not science. The science is settled. What is standing in the way is inertia. Routine labs that do not include ApoB. Physicians who do not order Lp(a). Patients who feel fine and assume fine means healthy. Feeling fine is not a cardiac clearance.
❌ Heart disease is not something that just happens to people.
❌ Your family history is not your destiny.
❌ Waiting for symptoms is not a prevention strategy.
The tools with the strongest data are targeted, available, and require your participation.
❤️ Bottom line:
Heart disease is not inevitable. It is a process we can interrupt. The evidence from JUPITER, FOURIER, ORION-10, and SELECT alone covers over 80,000 patients. This is not theory. Know your ApoB. Know your Lp(a). Know your hsCRP. Treat early and treat to target. A patient who acts at 40 does not become a patient who suffers at 60. The question I ask every patient now is simple: do you want to prevent your first event or survive your second one?
