⚕️Spinal vs. General Anesthesia for Hip Fracture Surgery: A Systematic Review & Meta-Analysis

One of the most common occurrences in the orthopaedic OR? Hip fracture.
With aging populations around the world, the incidence of hip fractures is only set to increase. But what's the best type of anesthesia to use during hip fracture surgery?
We conducted a systematic review comparing general vs. spinal anesthesia to find out!
🏗️METHODS
We searched MEDLINE, EMBASE, Cochrane, and the OE database for RCTs comparing spinal anesthesia vs. general anesthesia in hip fracture patients.
Outcomes of interest included:
1⃣ Blood loss
2⃣ 60-day mortality
3⃣ Duration of hospitalization
4⃣ Opioid consumption
5⃣ Incidence of delirium
6⃣ Incidence of serious adverse events
🔎RESULTS
We included 11 RCTs with a total of 2,359 patients across all studies!
Blood Loss
Patients who received spinal anesthesia had significantly less intra-operative blood loss compared to those who received general anesthesia!
60-Day Mortality
The risk of mortality was not significantly different between the two groups.
Duration of Hospitalization
Spinal anesthesia patients spent significantly less time in the hospital compared to general anesthesia patients.
Opioid Consumption
Patients who received spinal anesthesia used significantly less opioids compared to general anesthesia patients!
Adverse Events
The risk of delirium (p=0.77) and serious adverse events (p=0.88) were not significantly different between the two groups.
✅INTERPRETATION
In hip fracture surgery, spinal anesthesia was superior to general anesthesia, leading to less blood loss, fewer days of hospitalization, and less opioid consumption!

This is Charles Best and Frederick Banting. They were Canadian scientists that won the Nobel Prize for the discovery of insulin in 1923.
Banting was 32 when he received the Nobel Prize and he chose to share half the prize money with Best who was his assistant and just 24 years old at the time.
Banting refused to put his name on the patent and instead sold it to the University of Toronto for $1. He thought it was unethical to profit from a discovery that would save millions of lives. "Insulin belongs to the world, not to me," he said.
Unfortunately, the drug today has been subjected to price gouging by pharmaceutical companies with the average price per month rising to $450 in 2016.
