@DGlaucomflecken Or…sometimes what was originally scheduled as a 25 min annual exam turns into exploring new chest pain that may be something serious. 1-2 visits like this in a clinic day will almost certainly mean I am running behind.
@DGlaucomflecken The other part that is sometimes not apparent to patients…sometimes someone is late, and we don’t turn them away (because we know unanticipated things happen to make someone late to arrive), but now I am running behind for subsequent patients. This compounds as day goes on.
@PaulNWilliamz This is always hard for me. Recently I ask, “what weight are you aiming for?” and then follow up with “why?” Sometimes I learn about prior weights, sometimes I learn about other goals. Sometimes it’s less helpful.
How much should a cardiologist (who has debt, expenses, & doesn’t start until ~ age 34) make for placing a stent & stopping a heart attack (2 am/pm doesn’t matter)?
$674
Not asking for more. But people should know we only get 2% of that bill you get. We’re not the enemy.
@almoskow@rbganatra @zahirkanjee @BIDMC_Medicine Another caveat, from a PCP perspective, is that insurance companies are slow to respond to this data...I have many patients who would benefit from this med class but can’t afford the price.
@MKIttlesonMD Thanks for all the discussion so far!
In this patient, since we are limited on lipid lowering agents, what are thoughts on ASA 81mg daily?
What about same story/numbers but age is 45?
On retiring I realized just how many of my patients had unsolvable problems. On occasion I felt the need to apologize for not being able to do more. Most said they were just grateful I was there to listen and support them. That is the value of primary care https://t.co/IB7yRyrMIE
@laxswamy Are there ever issues with insurance coverage for a patient that has already gotten the vaccine? I haven’t had any issues yet with this, but I’ve wondered and had patients ask me about it when we aren’t sure about vaccine history.
1/ "If doctors and nurses #clocked out when their paid #hours were finished, the effect on patients would be calamitous. Doctors and nurses know this, which is why they don’t shirk.
The system knows it, too, and takes advantage." @danielleofri https://t.co/t0PtzYdN5O
