Despite the growth of endovascular treatments, microsurgical expertise remains necessary for certain neurovascular pathologies.

This case represents one of those.

40 year-old, smoker, with a large right MCA bifurcation aneurysm. Given morphology along with the angle/origin of the M2 divisions, endovascular intervention was felt to be sub-optimal and potentially riskier. He underwent microsurgical clip reconstruction and remains recurrence free at 3 years. For younger patients, durability and long-term recurrence are critical decision making factors. As such, clipping remains an excellent option for select patients.
