ICU pearls: When you come on service + you are told that a pt "failed PS trial" the previous day, try to find out what exactly happened. In several cases, the pt had just become apneic... This is not "weaning failure", it is just failure from our part to stop the sedation early!
@mtbastin Many days I don’t even know how I make it through! Our bodies are so incredibly amazing. Somehow, most days, we are able to push through the exhaustion and be the mom, pharmacist, wife, daughter, friend, etc we need/want to be but it shouldn’t be that way.

I would take it a step further and say: Some aspirations don’t even need abx at all. Aspiration does not automatically mean a bacterial PNA, it can just be a chemical pneumonitis. Less is more. #zentensivist#antibioticstewardship
