@BoesingLoretta@MomMom2seven It's serious because everything is serious when handling medications. But it's comparatively low risk compared to other types of errors that could happen within workflow because it's easily identifiable.
@BoesingLoretta@MomMom2seven In that scenario, you notify both patients and fix the error. But if the patient took the wrong person's medication leading to a potential adverse effect, that would require reporting. Each state has variations, but reporting requirements do exist.
@BoesingLoretta@MomMom2seven It varies by state & depends on the type of error. Most boards of pharmacy do require reporting of serious errors. An example of a non-serious error that may not require reporting would be accidentally giving a patient the wrong bag containing another person's medication. (1/2)
