💉 Recombinant zoster vaccine in inflammatory rheumatic diseases
Patients with IRDs have higher herpes zoster risk, especially with:
✅ High-dose glucocorticoids
✅ bDMARDs
✅ JAK inhibitors
✅ Combination immunosuppression
This review supports 2-dose RZV use in at-risk adults with IRDs.
Flare risk appears low in trials and cohorts, while pharmacovigilance data suggest short-term monitoring, especially in SLE.

BAFF inhibition may rebalance immunity in paediatric lupus nephritis.
In 60 children with class III/IV LN, belimumab plus standard therapy was associated with:
✅ Increased C3
✅ Reduced anti-dsDNA
✅ Decreased proteinuria
✅ Lower steroid exposure
✅ Expansion of IL-10-producing Bregs
✅ Reduced circulating and renal ILC1s
✅ Downregulation of IFN-stimulated genes
Belimumab may act beyond B-cell survival by restoring regulatory immune balance.

🫁 Lupus-related serositis is highly relapsing.
In 100 patients with #SLE and 180 serositis episodes:
✅ 87.1% responded to initial anti-inflammatory therapy
✅ Response was similar with corticosteroid-based vs aspirin/colchicine regimens
✅ 46% experienced ≥1 relapse
✅ Initial corticosteroid use did not increase relapse risk
✅ Belimumab was associated with lower relapse risk
HR 0.34, 95% CI 0.14 to 0.84
Aspirin/colchicine and belimumab deserve further evaluation in tailored strategies.

Still’s disease/sJIA is highly heterogeneous.
In this 10-year cohort of 110 patients:
✅ MAS occurred in 36.7%
✅ Nearly half of MAS cases occurred at disease onset
✅ ILD occurred in 8.2%
✅ MAS and ILD frequently co-occurred
✅ Serositis at diagnosis was associated with later MAS
✅ Rash, higher ESR and older onset predicted a polycyclic course
Clinical features may help identify patients needing closer monitoring.

🦴 Spinal ligaments in #SpA: can ultrasound help?
This study developed and validated a landmark-based US protocol for thoracolumbar ligaments:
✅ Lateral costotransverse ligament
✅ Supraspinous ligament
✅ Iliolumbar ligament
In healthy controls, visualisation was feasible and reliability was moderate to high.
In patients with SpA, US detected inflammatory and structural entheseal lesions.
A new window for assessing axial inflammation beyond conventional imaging.

🪣 Ankylosing spondylitis carries broad renal risk: TriNetX Global cohort (PS-matched)
📊 Adults with AS vs 1:1 matched controls, electronic health records
📈 CKD risk >2× higher in AS (HR 2.51, 95% CI 2.21–2.84)
⚠️ Dialysis initiation: HR 7.58 (6.19–9.27); AKI HR 2.18; glomerular disease HR 2.06
🔍 Authors call for routine nephrological surveillance in rheumatology pathways

🧠 Emotional well-being early in #JIA care may signal later mental health risk
✅ Moderate to severe #depressive symptoms were seen in 13% of patients vs 7% of controls
✅ Moderate to severe #anxiety symptoms were seen in 10% vs 2%
✅ Later mental health problems were linked more strongly to worse patient-reported outcomes than to physician-assessed disease activity
✅ Reduced emotional functioning near diagnosis may help identify patients who need early support
Routine HRQoL and mental well-being assessment should be part of JIA care.

🖥️ Ultrasound synovitis scoring, 10 years on
The #EULAR-OMERACT consensus-based scoring system has matured into a validated tool for both routine care and clinical trials.
✅ Reliable
✅ Sensitive to change
✅ Correlates with histological inflammation
✅ Predicts flare / supports bDMARD tapering decisions
✅ Feasible: 24-joint assessment in ~20 min
A strong case for standardised US assessment in arthritis.

Lateral elbow enthesitis (common extensor tendon) in a patient with ankylosing spondylitis on a TNF inhibitor.
Pearl:
Not all OMERACT entheseal elementary lesions are equally important!
-More specific 👉 erosions and 🔥power Doppler signal (especially ≤2 mm from cortex)
-Less specific 👉 heterogeneity, calcifications and enthesophytes (can occur due to biomechanical loading/stress, esp. LE enthesis with high BMI)
