Metastasis to thyroid -Common Pitfalls
- Metastatic RCC misdiagnosed as primary follicular neoplasm
- Metastatic well-differentiated adenocarcinoma as papillary thyroid carcinoma, for example metastatic lung adenocarcinoma (see pic 2-4)
- Metastatic high grade carcinoma as poorly differentiated thyroid carcinoma (PDTC) or anaplastic thyroid carcinoma

***Intranodal thyroid inclusions***
1. Predominantly smooth luminal borders
2. Absence of nuclear pseudoinclusions
3. Nuclear diameter not larger that two times of an adjacent lymphocyte
4. Negative for BRAF, VE1, HBME-1 and galectin-3 IHC
They can coexist with metastatic PTC
