Robotic Total Knee Arthroplasty -- A Randomized Trial

Robotic-assisted TKA could change how knee replacements are performed worldwide. However, more robust evidence is needed to confirm their effectiveness in relation to conventional methods.
This RCT aimed to assess whether robotic TKA can improve knee function, quality of life, and patient satisfaction compared to manual TKA.
๐๏ธ METHODS
100 patients with end-stage knee osteoarthritis were randomized to receive:
1โฃ Robotic TKA, assisted by the MAKO Robot (n=50)
2โฃ Conventional Manual TKA (n=50)
The primary outcome of interest was the change in knee function, as measured by the WOMAC function sub-score.
Additional outcomes include joint-specific scores, quality of life, patient expectations, and patient satisfaction.
๐ RESULTS
46 patients robotic TKA group and 41 in the manual TKA group were followed up at 6 months.
Knee function or joint scores were not statistically or clinically different between the two groups at 2 or 6 months!
However, a significantly greater improvement in pain scores at 2 months was observed in the robotic TKA group (although it was not a clinically meaningful difference)! The likelihood of achieving a clinically significant improvement in pain was also higher in the robotic TKA group.
No differences in quality of life, patient expectations or satisfaction were observed. However, the manual TKA had a significantly higher rate of ongoing knee pain at 6 months!
โ INTERPRETATION
No differences in function, quality of life, patient expectation and patient satisfaction were observed between robotic TKA and manual TKA up to 6 months post-operation!
However, pain scores were significantly better in the robotic TKA group (though not clinically meaningful), and the robotic TKA group had a greater likelihood of clinically important pain improvement and a lower rate of ongoing knee pain!
The results suggest that robotic TKA may not be much better than manual TKA, providing very little advantage in clinical outcomes!
