You need to look at the method used by Medicare to CERTIFY new providers.

Ex - In Calif, Medicare relies on the Calif. Depart of Health Agency that does licensing of providers to do the Medicare certification of those same providers.

If Calif is going to grant them a license, do you expect that Calif would deny them Medicare certification??

The licensing process is a joke -- that's how you end up having to kick 1000 Medicare Hospice providers out of the program.

Certification MUST BE INDEPENDENT of the States. The States are interested parties as they have a financial interest in having an ever-increasing amount of Medicare/Medicaid money coming into their economies.
