#CPRL2019@JonathanGreen01 balancing accountability with maximizing learning. Refocusing investigations based on data to stop the mistakes happening in the first place. Use learning as a focus on prevention. Look to performance and systemic issues, not just at conduct.
Some critics say we’ve not held officers to account. We disagree. We have interviewed officers, challenged decisions, questioned judgment calls, and reviewed over 2000 documents - although mistakes were made,no evidence officers deliberately misled a Judge in search warrant apps.
Our investigation of applications for search warrants made by @metpoliceuk as part of Operation Midland has identified shortcomings and organisational failings, with 16 recommendations made to change policing practice.

@MelDawson3@notlob99@NWalesPoliceFed We hold listening sessions with families who’ve had involvement in the complaints system, regular meetings with community groups & invite family members to attend debrief/learning meetings at end of investigations. Their input key in our end to end reform of complaints process.

Evidence of a positive collaborative relationship between @policeconduct and PSDs and @PFEW_HQ in Wales. This can only be to the benefit of officers and the public. We don’t agree on everything but large areas where we do.
We’ve just published our report on deaths during or following police contact in 2018/19. While it is very sad reading, understanding the types of deaths and common factors can help identify learning to prevent future incidents
