Gastric Hamartomatous Inverted Polyp(GHIP)

・粘膜下層を主体に異所性に腺管が増生し、胃内腔に膨張性に発育しポリープ状の形態を示す病変
・有茎性のポリープ様発育を示す型と、SEL/SMT様発育を示す型がある（後者の方が頻度としては高い）
→初期にはSEL/SMT様発育を示し、大きくなると病変の重みで粘膜が内腔側に引っ張られ、有茎性の形態を呈するとされる

・SEL/SMT型は比較的急峻な立ち上がり（病変の主座が粘膜深層～粘膜下層）
・病変の大部分は正常粘膜に覆われるが、頂部では内反および乳頭状増殖・過形成性変化をきたした腺窩上皮や、腺開口部様の陥凹を認める
・EUSでは第2～3層内を主座として不均一な内部エコーを示し、拡張した腺管や粘液貯留を反映した多発無エコー域が特徴的

症例は60歳代男性、胃体上部後壁のGHIPに対しCLEAN-NETを施行された1例
a. 胃体上部後壁のSMT、中央に陥凹（腺開口部）と境界不明瞭な発赤粘膜（腺窩上皮の過形成を反映）
b. EUSでは内部エコー不均一、多発無エコーを認める
c. 造影CTでは壁内発育型のSMT、内部は不均一な増強効果や嚢胞状変化（嚢胞状拡張を呈した腺管を反映）

・TFEB-altered renal cell carcinomaのうちの一つ(rearrangedとamplification)
・ rearrangedではbiphasic morphology with large clear cells and a nested architecture
・MelanA+, HMB45+(TFEB_amplificationではvariable。TFE3ではnegativeかfocal positive)
