The initial electrocardiographic (ECG) manifestation of hyperkalemia is the presence of peaked T waves. As hyperkalemia progresses, there may be loss of visible P waves, QRS widening, and ST segment elevation. A preterminal finding is a sinusoidal pattern on the ECG.
