Patients with Barrett’s oesophagus have a lifetime risk of oesophageal cancer between 3-13%. This is usually associated with reflux and hiatus hernia (seen in this case).

This patient has Barrett’s oesophagus with a deep ulcer with rolled edges and borders showing irregular mucosa and vessels.

This should ideally be washed and inspected closer with chromoendoscopy to give better pictures, but is indicative of an invasive cancer.

Interestingly, most Barrett’s cancer occur in the right side of the clock face (12 to 6 o’clock) due to mucosal damage from acid reflux.

Sadly, the patient had an urgent CT scan that showed liver and nodal metastases and is scheduled for palliative chemo.

PPIs won’t help in this case as this is a malignant ulcer caused by cancer infiltration, and neither will H pylori eradication. A follow-up OGD in 6 weeks would lead to delays in treatment.

So if you see an ulcer in a Barrett’s segment, consider cancer until proven otherwise.
