1/3 Normal Birth Ideology – Are We Asking the Wrong Question?
A Midwife's Perspective
Over the past week, the phrase "normal birth ideology" has dominated headlines following the publication of the Baroness Amos report and comments by Baroness Amos and Dr Bill Kirkup.
As someone who completed a BSc (Hons) Midwifery, practised as an NHS midwife and worked within NHS maternity services, I feel compelled to offer a perspective that appears to be missing.
I have never been taught normal birth ideology. I have never attended mandatory training promoting it.
I have never witnessed colleagues discussing or practising what is now being described as normal birth ideology.
That is not to say poor care has not occurred. It has. Women have died. Babies have died.
Thousands of families continue to live with avoidable harm, birth trauma and life-changing injury.
Those tragedies are undeniable. Every family deserves answers, accountability and, above all, the truth.
But truth demands precision.
It demands evidence.
It demands that we ask the right questions.
What do we actually mean by "ideology"?
The word ideology was coined in 1796 by the French philosopher Antoine Destutt de Tracy. Originally, it referred to the "science of ideas" the philosophical study of how ideas and knowledge are formed through experience.
During the nineteenth and twentieth centuries its meaning evolved. Following the writings of Karl Marx and Karl Mannheim, ideology came to describe a systematic set of beliefs through which the world is interpreted.
Political philosopher D. D. Raphael later described ideology as:
“A prescriptive doctrine that is not supported by rational argument." That distinction matters.
Calling something an ideology is very different from describing a clinical practice or evidence-based approach. It suggests decisions are driven primarily by belief rather than evidence, clinical judgement or professional standards.
As a qualified midwife, I was never taught that. I was taught anatomy.
Physiology.
Pathophysiology.
Pharmacology.
Obstetric emergencies.
Clinical assessment.
Communication.
Safeguarding.
Evidence-based practice.
Professional accountability.
Most importantly, I was taught to practise within the NMC Code, my professional scope of practice, NHS Trust policies and clinical guidelines.
Throughout university and NHS practice, birth was described using recognised clinical terminology.
A spontaneous vaginal birth occurs without medical intervention.
A normal vaginal birth may involve some intervention while remaining a vaginal birth.
An assisted vaginal birth involves forceps or ventouse.
A caesarean birth is an operative birth.
These are clinical descriptions.
They are not ideologies.
Pregnancy is a physiological process.
Women, by biological design, have the reproductive anatomy that enables pregnancy and, in many cases, vaginal birth.
Recognising that biological reality should not be controversial.
Acknowledging physiology does not deny that complications occur or diminish the importance of caesarean or assisted birth when clinically indicated.
Good maternity care depends on recognising when physiology remains normal and when pathology develops.
That is what midwives are trained to do.
The missing conversation
What concerns me most is not the discussion about birth.
It is the discussion we are not having.
Throughout my NHS career, I did not arrive at work making decisions based on personal philosophy.
Like every registered midwife, I practised within:
• The NMC Code
• Professional scope of practice
• Local NHS Trust policies
• Clinical guidelines
• National guidance
• Risk assessments
• Governance frameworks
• Escalation pathways
• Multidisciplinary decision-making
Midwives are often described as autonomous practitioners.
